Functional Medicine Certified team focused on a holistic approach.

Florida Residents Only.

Dr. Chris LeMay

My journey in root cause medicine began when I had several family members that were failed by the conventional medical model of "match the pill to the ill". I truly believe that it is our responsibility to offer people more. More than just a diagnosis and a prescription. More than 7 minute, impersonal medical visits. I am grateful to be part of the team at Index, where we offer personalized, progressive care and get to cheer on the progress of our members!

Holly J Niles

My personal and professional journey has taught me that the body is designed to heal itself. Getting to the "root cause", the "why" we get out of balance clears the path to wellness.  Functional medicine is the art of synergizing tools like good nutrition, eliminating toxins, positive mental and emotional health, deep breathing and the balance of rest and exercise. That synergy is the bridge to healing.

asked questions.

Where is Index Health available?

At the current time, Index Health is only available in the state of Florida. Stay tuned for updates on new availabilities across the USA and abroad. Follow us on social-media for updates: @IndexHealthUSA

Can I use my HSA/FSA to pay for Index Health?

Index Health membership is considered a "medically eligible" expense, so as such, can be paid for via an HSA/FSA.

Does Index Health accept insurance, Medicare or Medicaid?

We are able to care for individuals that are eligible to receive Medicare/Medicaid benefits, however, the cost of care received from Index Health is not able to be billed to Medicare/Medicaid by us, or the member. In this case we will require you to sign an ABN (Advance Beneficiary Notice of Non-coverage), which will waive your right to bill Medicare or Medicaid.

What if I change my mind?  Can I cancel my trial or membership?

Trial Cancelation: You can cancel the 30 day Trial anytime within the 30 days of the trial by simply emailing us at We will reimburse you in full ($99) if you have not used any of our services offered during the trial (physician or nutritionist visits). However, if you have used our medical visits during the trial, you will not be reimbursed for the $99. If you decide to continue as an Index Health Member, you will automatically be upgraded to the Index Health 12 Month Membership after 30 days.

Membership Cancelation: If you would like to cancel the Index Health 12 Month Membership upgrade, simply email us at However, if you’d like to cancel your membership early you will be responsible for paying for the costs of services that you have used. We will collect the balance between membership fees paid and fees associated with visits used according to the following schedule: $350 per 60 minute physician visit, $275 per 60 minute nutritionist visit.

Can Index Health be a primary care physician for me?

Since we are completely telemedicine at the current time, we encourage our members to maintain a primary care doctor, to ensure you have periodic, in-person physical examinations. Additionally, we operate more along the lines of "specialist" care, and frequently communicate with primary care doctors, to maximize the benefit of our care plans.

What's not included in my plan?

The Index Health membership covers your appointments with your dedicated doctor and nutritionist. Anything ordered outside of that will depend on your insurance for coverage. An example of this is our ordering of labs through national labs like Quest/LabCorp/Bioreference Labs. The potential cost to you is subject to your insurance's unique coverage details. If you are uninsured, we do have negotiated rates for self pay costs.

Are the advanced labs monitored for quality assurance?

Yes, the advanced lab companies that we use are all federally licensed under the Centers for Medicare & Medicaid Services Clinical Laboratory Improvement Amendments service (CLIA).

Can children join Index Health?

While we typically care for members above the age of 18, in certain situations, we can provide care for those who are younger than 18.  Please email us at and let you know if we are able to serve you/your child.

Are your doctors able to use genomics as part of my testing?

We use a variety of different genomics panels, each with different price points. For more pricing details, please discuss this with your Index Health doctor or email us at for more details.
