Polycystic ovarian syndrome (PCOS).

PCOS is a hormonal disorder that often leads to prolonged periods and/or excess androgen levels, often showing multiple small fluid collections on the ovaries.

Key focuses with our team at Index may include assessing for insulin resistance, abnormal adrenal/cortisol levels, gut health changes, as well as the consideration of detailed hormone cycle testing, then tailoring a treatment plan based off the findings.  

Premenstrual syndrome.

PMS is defined as a combination of symptoms that women often get about 1-2 weeks before their period, with common symptoms including bloating, headaches, food cravings, irritability, fatigue, and/or moodiness.

Our team will help you to understand if more advanced testing such as hormonal cycle testing, or detailed metabolic testing would be helpful. We then create a plan designed just for you, based on the detailed results that we have gathered.

Uterine fibroids.

Uterine fibroids are benign growths of the uterus, that often lead to longer than normal menstrual bleeding, heavier bleeding, and pelvic pain.

Some common testing focuses include comprehensive hormone cycle testing, insulin resistance panels, detailed nutrient panels, and toxin panels, amongst others. The personalized treatment plan is aimed at the detailed findings.


Endometriosis is a condition that involves tissue from the uterus growing outside the uterus, leading to symptoms such as painful periods, painful bowel movements or urination, excessive bleeding, and it can also be a cause of infertility.

For workup approaches and treatment planning purposes, we may discuss the value of a month long hormone cycle test, and will also often screen for the possibility of systemic inflammation, stress (DHEA/cortisol testing) and sleep abnormalities (including wearable trackers), amongst others.

Thyroid health.

TSH, Free T3, and Free T4, Anti-TPO and thyroglobulin antibodies, and Nutrient markers needed for thyroid hormone production (B vitamins, Vit D, selenium, zinc, iodine, iron, etc.).

Energy levels / Fatigue.

CBC/CMP, GGT, comprehensive nutrient assessment for those needed for ATP production (mitochondria health), chronic infection testing, consider toxic burden assessment (mold, mercury, testing at times).

Personalization is our policy. These tests & screenings tell us the current status of a woman’s hormonal health and which areas to focus on to optimize women’s health, including identifiable causes for low energy, fatigue, or brain fog.

What’s in your personalized functional medicine plan?

We aim to provide women with holistic care that lets them be confident in their health.

Nutrition plan (varies).

Elimination plan with food reintroduction with tracking, mito food plan, detox food plan, cardiometabolic food plan, consideration of intermittent fasting based on genetic insights.

Lifestyle and movement plan.

Supplements and medication.

We believe in a natural, nutrition-first approach. As a policy, we do not make any money from supplements, and supplement recommendations are targeted entirely at the key focus points from the clinical discussions and the lab results. Our physicians do utilize prescriptions. However, it is infrequent.

Where is Index Health available?

Can I use Index Health for Advanced Primary Care?

Yes. Advanced Primary Care at Index Health focuses on prevention and health planning using personalized health plans from genomics and lab test data. Your physician may recommend that you see a specialist for certain conditions. We're available from 9:00 AM-5:00 PM Monday's through Thursdays, and 9:00 AM - 3:00 PM on Fridays.  

Can I use my HSA/FSA to pay for Index Health?

Yes. Index Health membership is considered a medically eligible expense therefore, it can be paid for via an HSA/FSA.

Does Index Health accept insurance, Medicare or Medicaid?

We can provide functional medicine help for patients eligible to receive Medicare/Medicaid benefits; however, the cost of patient treatment received from Index Health cannot be billed to Medicare/Medicaid by the member holding the insurance policy or us. In this case, we will require you to sign an ABN (Advance Beneficiary Notice of Non-coverage), which will waive your right to bill Medicare or Medicaid.

What's the cancellation policy?

At Index Health, our membership is set up as an ongoing plan with the option to pay in monthly installments or in a single upfront payment. All members have the opportunity to cancel their membership at any point in their agreement.

Option 1 - Single monthly membership - $179/month
Option 2 - Single yearly membership - $1,800/year (save $348 with the yearly membership)
Option 3 - Family monthly membership - $149/month per person, minimum two people.
Option 4 - Family yearly membership - $1,600/year per person, minimum two people.
Option 5 - Single appointments - physician $350, or $275 nutritionist

If you make monthly payments on your membership and you would like to cancel, we will invoice you for the services you have used, but not paid for yet after deducting how much you have already paid.

As an example, if you’ve paid for 3 months, or $537 ($179 + 179 + 179) and used 2 doctors visits (which we individually charge $350 per hour) we will invoice you a remaining $187.

If you paid for your membership upfront, you will not be charged a final fee to cancel. You will have access to Index Health services through your cancellation date.
