Yes. Advanced Primary Care at Index Health focuses on prevention and health planning using personalized health plans from genomics and lab test data. Your physician may recommend that you see a specialist for certain conditions. We're available from 9:00 AM-5:00 PM Monday's through Thursdays, and 9:00 AM - 3:00 PM on Fridays.
Yes. Index Health membership is considered a medically eligible expense therefore, it can be paid for via an HSA/FSA.
We can provide functional medicine help for patients eligible to receive Medicare/Medicaid benefits; however, the cost of patient treatment received from Index Health cannot be billed to Medicare/Medicaid by the member holding the insurance policy or us. In this case, we will require you to sign an ABN (Advance Beneficiary Notice of Non-coverage), which will waive your right to bill Medicare or Medicaid.
At Index Health, our membership is set up as an ongoing plan with the option to pay in monthly installments or in a single upfront payment. All members have the opportunity to cancel their membership at any point in their agreement.
Option 1 - Single monthly membership - $179/month
Option 2 - Single yearly membership - $1,800/year (save $348 with the yearly membership)
Option 3 - Family monthly membership - $149/month per person, minimum two people.
Option 4 - Family yearly membership - $1,600/year per person, minimum two people.
Option 5 - Single appointments - physician $350, or $275 nutritionist
If you make monthly payments on your membership and you would like to cancel, we will invoice you for the services you have used, but not paid for yet after deducting how much you have already paid.
As an example, if you’ve paid for 3 months, or $457 ($99 + 179 + 179) and used 2 doctors visits (which we individually charge $350 per hour) we will invoice you a remaining $243.
If you paid for your membership upfront, you will not be charged a final fee to cancel. You will have access to Index Health services through your cancellation date.
