You will receive an email from us to book a Discovery Call with one our Care Managers. This initial intake call will be used to review all new patient intake items you have provided and book your first functional medicine appointment.

You will also receive an email from "Cerbo" to activate your patient account and gain access to your patient dashboard.

After logging in, please complete all medical history forms, and upload past labs (if applicable).
