You'll get an email to book an Onboarding Call with our Care Manager. During this call, we'll review your intake items and schedule your first functional medicine appointment.
You will also receive an email from "Ziemba Medical P.C." to activate your patient account and gain access to your patient dashboard.
After logging in, please complete all medical history forms, and upload past labs (if applicable).
