See patients via telemedicine and complete online charts within 24 hrs within EMR.
Provide excellent, personalized patient care within your scope of training.
Become a teammate with our already established, very experienced, IFM trained team of physicians and nutritionists to help us serve the people we care for, to optimize their outcomes and achieve their health and wellness goals.
Help our members to implement the care plans that are put forth by our physicians and nutritionists. Assist them with health literacy, reducing lifestyle risks with proactive health planning, and supporting their readiness to change.
Empower individuals to become active in their own health by using motivational interviewing, mind/body interventions, and behavior change techniques to make ongoing healthy lifestyle choices.
Participate in patient care program development (i.e. 90 day behavior change).
Assist in the answering of patient messaging inquiries and requests.
Go the “extra mile”, to assist when members need a specific resource, referral, or guidance.
Triage appropriately and communicate, if you realize that a member concern needs to be addressed acutely by our physicians or nutritionists.
Actively participate in committees, projects, company initiatives.
Work collaboratively with care managers to help guide scheduling guidelines/panel management for the health coaching aspects of care.
Monitor program engagement, ongoing staff development, and maintenance of team goals.
Act as an exemplary mentor for fellow colleagues, to maintain a highly functioning team.
Maintain ongoing medical education, to keep current with evolving medical understandings, and the “state of the art” care.
Help guide standards, and adherence to those standards for the health coaching team.
Represent Index Health well in the local and broader community.
Follow organizational policies, procedures, and protocols.
Perform other duties as directed or required.
Educational degree in a health related field (i.e. Nutrition, Exercise Physiology, Public Health).
Functional Medicine Coaching Academy or Institute for Integrative Nutrition training preferred.
National Board Certified Health and Wellness Coach certified is preferred, but not necessary.
3+ years of experience in relevant professional work experience.
Strong communication skills (verbal, written, presentation, interpersonal) with all types/levels of audience.
Strong time management and organizational skills.
Ability to deliver content in an engaging way and adapt to audience needs and/or questions.
Demonstrated history of operating well in a team environment.
Experience managing health populations with diverse acuity needs.
Flexible schedule to accommodate scheduling needs for our members (evening efforts on occasion (likely for group sessions), weekends are not expected).
Telemedicine experience a plus, with the ability to navigate multiple application and tools to support remote patient care (i.e., Athena (EMR), Active Campaign, Slack, Doxy.me).
Other preferred, but not necessary backgrounds include prior group coaching, multiple languages spoken.
